The proposed changes are:
- to relocate services from The Welby Practice, Bottesford to the adjacent Belvoir Vale site
- to modify the opening hours of our branch surgery at Waltham
NHS Long Term Plan - As medicine advances, health needs change and society develops, the NHS has to continually move forward so that in 10 year’s time we have a service fit for the future.
The NHS Long Term Plan is drawn up by frontline staff, patients groups, and national experts to be ambitious but realistic.

Evening Clinic - Extended Access Service
From week commencing 26 November 2018 the above service will be delivered at:-
Belvoir Vale Surgery, Bottesford 6.30pm-8.30pm Thursday
There will be a clinician session and nursing session running with appointment availability at an increased level to accommodate the same level of appointments previously offered.
The last Monday clinic will take place on 19 November 2018.
We will assess the level of uptake over the coming months.

As there are two sites in Bottesford we will refer to the practice as The Welby Practice with the site name as Belvoir Vale or Welby Bottesford, the other locations are known as Harlaxton and Waltham.
You will notice some changes e.g. The Welby Practice header on letters, etc.
You will be able to book appointments with GPs, ANP, NP, Nurses and HCAs across all the sites.
Repeat prescriptions are recorded on our clinical system and can be re-ordered as required. We do request that we are given a minimum of two full working days notice when ordering repeat prescriptions so that medication can be safely dispensed. In accordance with NHS guidance repeat prescriptions are, in general, issued as a one month supply only. Repeat prescriptions can be ordered in the following ways:
If you are ordering by telephone, you must call during working hours.
For urgent prescriptions, please contact the Practice during our opening hours and we will endeavour to arrange these for you. If you require urgent medication or have a query outside of our opening hours, please contact NHS 111.

*PLEASE REMEMBER TO ALLOW TWO FULL WORKING DAYS FOR YOUR REPEAT MEDICATION TO BE PROCESSED AND TAKE WEEKENDS AND BANK HOLIDAYS INTO ACCOUNT*
Patients on repeat medication may be asked to see a doctor, nurse practitioner, practice nurse or clinical pharmacist at least once a year to review their regular medications. A notification when this is due should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs). The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication.
These charges apply in England only. In Northern Ireland, Scotland and Wales prescriptions are free of charge.
If you will have to pay for four or more prescription items in three months, or more than 15 items in 12 months, you may find it cheaper to buy a PPC.

Please allow two full working days, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the Practice.
This enables prescribers to send prescriptions electronically to local or distant pharmacists - there is no longer a requirement for a paper prescription. There are a few exceptions to the service which include controlled drug prescriptions which have to be on a paper prescription for now.
Initially we will nominate the pharmacy where you have already told us you want to collect prescriptions from. You can change this easily by telling us that you want to collect from a new location when your next prescription is due, or ask the person prescribing the medication before the medication is issued. You can nominate a separate supplier for stoma products.
Dispensary patients are not affected by this service.
Patients may be asked by pharmacists to sign up to this or any other service. If you do it is important to understand the implications FOR EXISTING DISPENSING PATIENTS - ie for patients receiving their medications from our dispensary. This message does not apply to patients who live within a mile of a pharmacy as we are prevented by law from offering a service to these patients.
The EPS2 service will NOT include our GP dispensing services as dispensing doctors have been EXCLUDED from the service for the moment.
If you are a dispensing patient, once you have signed up to ANY pharmacy service YOU WILL NOT BE ABLE TO CHANGE BACK TO A GP DISPENSING SERVICE without making an application to the PCT which will only be considered under exceptional circumstances and this may be rejected.
If you currently use our dispensary DO NOT SIGN UP to any commercial pharmacy services unless you are sure you never wish to use our dispensary services again. The government has determined that you will not be allowed to change your mind later. We wish to continue to provide a comprehensive service to our eligible patients.
