The proposed changes are:
- to relocate services from The Welby Practice, Bottesford to the adjacent Belvoir Vale site
- to modify the opening hours of our branch surgery at Waltham
NHS Long Term Plan - As medicine advances, health needs change and society develops, the NHS has to continually move forward so that in 10 year’s time we have a service fit for the future.
The NHS Long Term Plan is drawn up by frontline staff, patients groups, and national experts to be ambitious but realistic.

Evening Clinic - Extended Access Service
From week commencing 26 November 2018 the above service will be delivered at:-
Belvoir Vale Surgery, Bottesford 6.30pm-8.30pm Thursday
There will be a clinician session and nursing session running with appointment availability at an increased level to accommodate the same level of appointments previously offered.
The last Monday clinic will take place on 19 November 2018.
We will assess the level of uptake over the coming months.

As there are two sites in Bottesford we will refer to the practice as The Welby Practice with the site name as Belvoir Vale or Welby Bottesford, the other locations are known as Harlaxton and Waltham.
You will notice some changes e.g. The Welby Practice header on letters, etc.
You will be able to book appointments with GPs, ANP, NP, Nurses and HCAs across all the sites.

Services available from your GP practice
Advanced Nurse Practitioners are highly skilled and can deal with most health conditions and illnesses. They can prescribe medications and refer patients for treatment from other health services.
Services available from your GP Practice
RGN Ba (Hons) Nurse Practitioner with Non-Medical Prescribing and a trainee in Advanced Clinical Practice (MSC) Rachel was born and raised in Nottingham. In 2000 she started her life long ambition of becoming a nurse and attended Sheffield Hallam University where she completed her Nursing Degree in 2004. Rachel has been working for the practice since February 2009 when she joined the team as a Practice Nurse. Rachel’s previous working background involved working as a Deputy Team Lead Staff Nurse on a Gynaecology Unit and then as a Staff Nurse in Genito Urinary Medicine. Rachel works as a prescribing Clinical Practitioner having completed her Non-Medical Prescribing in 2015 and holds regular clinics on a Monday and Tuesday. Rachel is Lead Nurse for the practice Nurses and Health Care Assistants and enjoys training and supporting these pivotal roles in primary care. Outside of work Rachel is happiest when spending time with her family and friends and is kept busy with her biggest project the family.
Practice nurses are qualified and registered nurses. They can help with health issues such as family planning, healthy living advice and dressings. The practice nurses manage long-term health conditions such as asthma or diabetes, hold minor ailment clinics and carry out cervical smears.
Our Nursing Team also includes our Health Care Assistants who are skilled and qualified in performing the following:-
COPD, diabetic and hypertension reviews (work-up), BP monitoring, skilled phlebotomists (blood taking), wound management, offer dietary advice, perform ECGs, carry out NHS Health Checks, spirometry, urinalysis, B12, flu, pneumonia and shingles vaccinations. Amanda is also qualified at performing ear irrigation.
Our Phlebotomist is here to take blood samples from patients so that we can monitor your care.

Receptionists provide an important link for patients with the practice and are your initial contact point for general enquiries. They can provide basic information on services and results and direct you to the right person depending on your health issue or query. Receptionists make most of the patient appointments with the GPs and nurses. They also perform other important tasks such as issuing repeat prescriptions and dealing with general enquiries, financial claims and also help manage ensuring patient records are up to date.
The community nursing service (commonly known as District Nurses) provides nursing care to people in their own homes. It is mainly for people who are confined to their home and who are over 18 years of age.
The team provides healthcare that enables individuals to increase and maintain their independence, while remaining at home.
They work closely with social services, GPs, other healthcare professionals and also patients and carers. The service makes sure that patients have the care, help and support they need, while maintaining their comfort and dignity.
There are physiotherapy sessions at Bottesford, Harlaxton and Waltham. Simply ask reception for an appointment with one of the doctors who will be able to refer you to our in-house service.
A community chiropodist regularly attends the Bottesford surgery. Ask at reception and complete a chiropody request form. Alternatively, please make an appointment to discuss with one of our GP’s.
